Consent and release
I, ________________________, consent to receiving a tattoo from the artist listed below at The Ink Vault Tattoo Studio.
I understand that:
- The tattoo process involves piercing the skin, which can cause pain, bleeding, infection, and scarring.
- Proper aftercare is essential for healing and preventing complications & will be explained.
- I have disclosed all health conditions and understand the risks involved.
- No guarantees have been made as to the final appearance or permanence of the tattoo.
- I release The Ink Vault Tattoo Studio, its owners, and employees from any and all liability related to this procedure.
Independent artist acknowledgment
All tattoo artists operating within The Ink Vault Tattoo Studio are independent contractors. The studio is not responsible for the actions, outcomes, or conduct of any artist. Any disputes or claims regarding tattoo services should be directed solely to the individual artist performing the work.
Design & placement confirmation
I have reviewed and approved the tattoo design, spelling, and placement. I confirm that the stencil or freehand placement has been proofread and positioned to my satisfaction. The artist is not liable to fix any errors free of charge.

Shop minimum & payment policy
- The shop’s minimum for any tattoo is $85.
- If a tattoo cannot be completed during the scheduled session, I am responsible for full payment and understand a continuation fee will apply for future appointments, unless the artist specifies otherwise for specific reasons (must be approved by owner).
